Social Worker – Transitions Team

Job Type: Full-Time

We are currently recruiting for an experienced Social Worker to join a Transitions Team supporting young people aged 14+ moving from children’s to adult services.

This role involves supporting young people who meet Care Act eligibility criteria, ensuring they receive appropriate, person-centred assessments and support plans to promote positive outcomes and smooth transitions into adulthood.

Key Responsibilities

• Screen referrals into the Transitions Team
• Complete Care Act assessments in line with statutory requirements
• Develop person-centred care and support plans
• Support young people transitioning from children’s to adult services
• Work collaboratively with families and multi-agency professionals
• Maintain accurate and up-to-date case records
• Promote positive outcomes and independence for young people

Requirements

• Qualified Social Worker with relevant registration
• Experience completing Care Act assessments
• Strong assessment and analytical skills
• Ability to work effectively with families and multi-disciplinary teams

Desirable Experience

• Previous experience working with Looked After Children (LAC)
• Experience within a Transitions or Disability Team
