Social Worker – Adult Social Care

We are seeking a dedicated and experienced Social Worker to join an Adult Social Care team supporting vulnerable adults to achieve independence, maintain relationships, and remain active members of their community.

This role involves working with adults, carers, families, and partner agencies to assess needs, develop support plans, and deliver safeguarding interventions in line with legislation and professional standards.

Key Responsibilities

• Assess the needs of vulnerable adults and develop person-centred support plans
• Promote independence, inclusion, and positive outcomes for service users
• Undertake safeguarding investigations and manage risk appropriately
• Work collaboratively with families, carers, and multi-agency professionals
• Manage a complex caseload and prioritise work effectively
• Undertake assessments, interviews, and observations using appropriate frameworks
• Coordinate, implement, and review support plans
• Maintain accurate records and reports using required systems
• Promote equality, diversity, and anti-discriminatory practice
• Operate within relevant legislation, policies, and professional standards

Requirements

• Qualified Social Worker with relevant registration
• Experience within Adult Social Care
• Strong safeguarding knowledge and risk management experience
• Ability to manage complex caseloads independently
• Excellent communication and report-writing skills
• Strong understanding of legislation and social care frameworks
• Full driving licence and access to a vehicle may be required
• DBS check required for this role
