We are seeking an experienced Qualified Social Worker to join an Adolescent Support Team supporting vulnerable young people aged 11–18 and their families.

This role focuses on safeguarding, prevention, and delivering high-quality interventions to support adolescents at risk of harm, homelessness, or entering the care system unnecessarily.

The successful candidate will work within a supportive multi-agency environment, helping young people achieve positive outcomes through strengths-based and solution-focused practice.

Key Responsibilities

  • Manage a caseload of complex adolescent cases
  • Complete assessments, planning, and intervention work
  • Undertake direct work with young people and families
  • Support 16–17-year-olds presenting as homeless
  • Complete Joint Housing Assessments with District Councils
  • Chair Child in Need meetings and coordinate support plans
  • Work collaboratively with CAMHS, Education, YOT, Police, Housing, and partner agencies
  • Promote safeguarding and positive outcomes for vulnerable young people
  • Utilise strengths-based and evidence-informed practice
  • Maintain accurate case records and reports

About the Team

The Adolescent Support Team is based in Gravesend and covers:

  • Swanley
  • Sevenoaks
  • Gravesham
  • Dartford

The team works closely with Social Work Assistants and other specialist services, offering a collaborative and supportive working environment.

Requirements

  • Qualified Social Worker (DIPSW, CQSW, CSS or equivalent)
  • Experience within Children’s Social Care and safeguarding services
  • Strong direct work and engagement skills with adolescents and families
  • Experience managing complex cases and crisis interventions
  • Strong communication and partnership working skills
  • Ability to work within multi-agency safeguarding environments
