We are seeking experienced Occupational Therapists to support a Reablement at Home service focused on helping individuals regain independence and achieve therapy-led goals within their own homes.

This role involves working within a multi-disciplinary team supporting adults through short-term, outcome-focused interventions designed to reduce long-term reliance on care services and improve wellbeing.

The service supports hospital discharge pathways and promotes a strengths-based approach that enables individuals to regain confidence and daily living skills.

Key Responsibilities

  • Carry out comprehensive Occupational Therapy assessments within the community
  • Support adults with complex and substantial needs to maximise independence
  • Develop and review therapy-led reablement plans
  • Assess and recommend specialist equipment and adaptations
  • Work collaboratively within a multi-disciplinary and neighbourhood-based team
  • Support safeguarding enquiries and risk management processes
  • Supervise Occupational Therapy Assistants and students where required
  • Maintain accurate records and reports using appropriate systems
  • Promote preventative and strengths-based practice
  • Work closely with health professionals, carers, and partner agencies

About the Service

The Reablement Service provides short-term interventions of up to 6 weeks, helping service users:

  • Regain daily living skills and confidence
  • Reduce dependency on long-term care services
  • Improve independence and wellbeing
  • Receive support within their own home environment

The service works closely with Occupational Therapists, Assistants, health services, and community partners to deliver outcome-focused care.

Requirements

  • Degree in Occupational Therapy
  • Experience completing Occupational Therapy assessments within Adult Social Care
  • Strong knowledge of reablement, rehabilitation, and preventative practice
  • Understanding of relevant legislation including the Care Act and Mental Capacity Act
  • Experience working within multi-disciplinary teams
  • Strong communication, assessment, and report-writing skills
  • Enhanced DBS required
